- Alternatively, you may request a pdf claim form, then submit via email or mail.

Claim Review:
- Our expert claims specialists will promptly review your submitted documentation.
- Additional information may be requested to accurately process your claim.

Claim Status Updates:
- Monitor your claim status anytime through our online portal. All you need is your Claim Number, First and Last Name.
- You’ll receive timely email notifications regarding important updates.
